Repair form Or RMA?
An RMA Service Form is used for bringing in your repair to Imagin. You will need to fill out the form, Print it (Ctrl P), then place it in the shipping box along with your repair item. After printing, then click the Submit button, this will automatically register you with us and it will send us a notification that you are sending in a repair.
Upon us receiving the repair item along with the provided RMA form, we then check it in and return E-mail you a statement that we have received your item.
So please keep an eye out and check your E-mails for all correspondence from ImaginRepair.com

FILL OUT THE SERVICE FORM FOLLOWING THESE BASIC INSTRUCTIONS
1. WHERE IT SAYS RMA# WE WILL USE YOUR OFFICE PHONE NUMBER WITHOUT HYPHENS (i.e. 6505961001)
2. PLEASE PROVIDE THE OFFICE NAME, OR DOCTOR'S NAME AND A CONTACT PERSON
3. A RETURN SHIPPING ADDRESS, WHERE TO RETURN SHIP YOUR ITEM TO AND ALSO STATE AN INSURANCE AMOUNT, IF NO AMOUNT IS STATED WE WILL INSURE ITEM FOR $1000 (IF REPAIRED). NOTE, WE ARE NOT RESPONSIBLE FOR LOST ITEMS AND OR COSTS THAT ARE HIGHER THAN THE INSURED AMOUNT STATED,
SO PLEASE MAKE SURE YOU FILL IN AN AMOUNT YOU’RE WILLING TO TAKE.
4. INCLUDE YOUR PHONE NUMBER AND AN E-MAIL ADDRESS, WE WILL USE EMAILS TO CONTACT YOU AND SEND YOU A QUOTATIONS OR ANYOTHER NOTIFICATIONS REGARDING YOUR REPAIR ITEM.
5. ALSO IMPORTANT, PLEASE MAKE SURE YOU STATE YOUR BRAND OF CAMERA / SENSOR, THE MODEL NO. & SERIAL NO. & AN ACCURATE DESCRIPTION OF THE PROBLEM YOU'RE HAVING.
6. ONE MORE THING, SO WE CAN DUPLICATE YOUR ISSUE, PLEASE PROVIDE US WITH YOUR COMPUTER TYPE, OPERATING SYSTEM (PC, WIN 7,8,10,11/MAC) AND THE IMAGING SOFTWARE PROGRAM YOU ARE USING.
7. WHEN COMPLETED, PRESS Ctrl P TO PRINT THIS FORM, THEN CLICK THE SUBMIT BUTTON AND A COPY OF THIS FORM WILL BE SENT TO US ACKKNOWLEDGING US YOUR ITEM IS ON ITS WAY. UPON US RECEIVING YOUR ITEM, WE WILL THEN RETURN AN EMAIL LETTING YOU KNOW WE HAVE RECEIVED IT WITH A DATE.
PLEASE REMEMBER TO NOTE YOUR TRACKING NUMBER WHEN USING A SHIPPING SERVICE AND INSURE YOUR ITEM FOR A VALUE YOU WILL CONSIDER. WE ALSO ASK THAT YOU PLEASE CHECK WITH YOUR SHIPPING COMPANY FIRST BEFORE CALLING US IF WE HAVE RECEIVED YOUR ITEM. MANY THX!
